I HERBY AUTHORIZE THE STAFF OF CRUSADERS BASEBALL CAMP TO ACT FOR ME ACCORDING TO THEIR BEST JUDGMENT IN AN EMERGENCY REQUIRING MEDICAL ATTENTION AND I HEREBY WAIVE AND RELEASE CRUSADERS BASEBALL CAMP FROM ANY AND ALL LIABILITY FOR AN INJURIES INCURRED WHILE ATTENDING THE BASEBALL CAMP.
